Eligibility

You are eligible to enroll in benefits if you are a regular full-time or regular part-time employee who works at least 20 hours per week. Your benefits become effective on your date of hire. You may also elect coverage for your eligible dependents in the medical, dental, and vision plans.

Eigible Dependents Include:

  • Your legal spouse or domestic partner
  • Your children up to age 26 who do not have employer-sponsored coverage of their own
  • Your children of any age who are physically or mentally disabled and incapable of supporting themselves and can be claimed as a dependent on your U.S. federal income tax return

Eligible Children Include:

  • Your natural children
  • Your legally adopted children
  • Children in your legal custody
  • Your stepchildren if living with you
  • Your domestic partner’s children
  • Any children covered under a Qualified Medical Child Support Order (QMCSO) as defined by applicable federal law and state insurance laws that apply to this plan
